CHN was created to provide a seamless continuum of post-acute and senior care services in a more cost-effective manner while collaborating to improve quality and reduce administrative costs.  We offer services and support to optimize the service capacity of our member organizations, which include senior living campuses offering Independent Living, subsidized and supportive housing, assisted living/personal care, and skilled nursing. 

CHN owns and operates a variety of ancillary business lines including Rehabilitation/Therapy, Home Health, Hospice, Group Purchasing, and Post-acute Care Consulting.
