Senior Care In Motion is a Covenant Health Network program dedicated to providing coordination of integrated health care services within our Continuing Care Retirement Communities. Coordination of care will assist the resident/patient to experience safe and quality care and services. Receiving well-coordinated and integrated care focuses on aging in place while providing services to improve and maintain present chronic conditions and therefore decreasing the opportunity for unnecessary hospitalizations.

The mission of Senior Care In Motion is dedicated to providing senior-focused models of care and member services that balance quality with efficiency to a growing membership of mission-driven senior care providers.

The vision of Senior Care in Motion is to provide a seamless continuum of post-acute and senior services in a more cost-effective manner, while collaborating to improve quality and health outcomes, and to reduce administrative costs. 

The goal of Senior Care In Motion is to promise care and services for the senior served which will be timely, safe, effective, patient-centered, efficient and equitable.
