Curana Health and Covenant Health Network Team Up to Transform Value-Based Care in Arizona

12 Facilities Enhance Primary and Post-Acute Care Services for Seniors Through Highly Integrated, High Touch Value-Based Care Programs

Austin, Texas and Phoenix, Arizona (July 17, 2024)Curana Health and Covenant Health Network (CHN) are pleased to announce a partnership that helps advance the delivery of integrated, value-based primary and post-acute care to residents in Arizona. Established in January 2024, this partnership has seen significant success in 12 initial CHN communities.

 

Through this collaboration, Curana Health provides physician-led care teams specializing in on-site primary care services (provided in resident rooms and dedicated clinic spaces). Curana also supports CHN’s Arizona facilities by enabling them to participate in Curana’s high-performing Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO), which is a program operated by the Centers for Medicare & Medicaid Services designed to enable high-quality, cost-effective, and coordinated care for Medicare beneficiaries.

On average, Curana’s partner organizations have experienced a more than 39 percent reduction in unnecessary hospitalizations.

 

CHN’s Arizona facilities are the first CHN facilities to integrate Curana Health’s value-based care programs, with an eye on expanding to additional markets later this year.
