Austin, Texas and Phoenix, Arizona (July 17, 2024) – Curana Health and Covenant Health Network (CHN) are pleased to announce a partnership that helps advance the delivery of integrated, value-based primary and post-acute care to residents in Arizona. Established in January 2024, this partnership has seen significant success in 12 initial CHN communities.

 

Through this collaboration, Curana Health provides physician-led care teams specializing in on-site primary care services (provided in resident rooms and dedicated clinic spaces). Curana also supports CHN’s Arizona facilities by enabling them to participate in Curana’s high-performing Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO), which is a program operated by the Centers for Medicare & Medicaid Services designed to enable high-quality, cost-effective, and coordinated care for Medicare beneficiaries.

CHN is a post-acute integrated care-delivery system serving non-profit and faith-based providers of senior care services for more than 12,000 seniors in multiple states.

Transforming the care experience for senior living residents is at the heart of Curana Health’s mission, and senior living communities that have implemented Curana’s primary care model have seen meaningful improvements in resident outcomes and experience. One partner organization saw a 55 percent decrease in falls resulting in injury and a 25 percent increase in resident happiness, while another saw a 43 percent reduction in polypharmacy. On average, Curana’s partner organizations have experienced a more than 39 percent reduction in unnecessary hospitalizations.

 

CHN’s Arizona facilities are the first CHN facilities to integrate Curana Health’s value-based care programs, with an eye on expanding to additional markets later this year.
