Covenant was created to provide a seamless continuum of post-acute and senior care services in a more cost effective manner, while collaborating to improve quality and reduce administrative costs. In addition to the service capacity of our member organizations which are typically comprised of a campus offering Independent Living, subsidized and supportive living housing, assisted living/personal care, and skilled nursing. CHN owns and operates a variety of ancillary business lines including a Rehabilitation/ Therapy Company, a Hospice organization, a Technology Development Company, a Long Term Care Pharmacy, an Insurance Broker as well as a Management Consulting Organization.
CHN has a fully dedicated Quality Improvement Department that benchmarks key metrics such as Hospital days per 1000, hospital re-admission rates, incidence of pressure ulcers, falls and other adverse events in addition to the Federal Quality Measures and the Five Star Rating. Best practice adoption and promulgation can consequently ensue. These systems have enabled the network to be a successful partner to a number of health plans and payers in Gain Share arrangements.
Covenant Health Network is actively pursuing potential partnering opportunities built around the Affordable Care Act and other health care reform initiatives. Reduction in avoidable hospitalization and acute care re-admissions, coupled with QAPI enabled outcomes management will yield significant cost savings in the form of federal incentives to care providers.
